Event Description
|
Complainant alleged that while attempting to cardiovert a (b)(6) male patient, an arc was heard from the electrode pads.Complainant indicated that there was no adverse effect to the patient due to the reported malfunction.

|
The pro padz solid gel radiolucent multifunction electrodes were returned to zoll medical corporation opened and adhered together.Visual inspection found a heavy presence of hair on the adhesive of the electrodes and discoloration on the conductive plates indicating the occurrence of discharge.The electrodes appear to be functional although sent to zoll in an unsealed package which dried out the gel.All indications suggest patient preparation was a factor.The electrode pads were scrapped.Zoll recommends that patients are cleaned and hair clipped prior to applying electrode pads to assure good coupling of electrode to skin contact.Poor adherence and/or air under the electrodes can lead to the possibility of arcing and skin burns.This investigation was closed as improper patient preparation prior to the application of the electrodes.Analysis of reports of this type has not identified an increase in trend.
