It was reported during an initial hip arthroplasty the thread on the straight inserter threaded shaft broke off in the cup of a g7.Surgeon could not remove it and felt comfortable with leaving it inside the threads of the center hole of the cup.Attempts have been made and additional information on the reported event is unavailable at this time.

This follow-up report is being submitted to relay additional information.Complaint sample was evaluated and the reported event could not be confirmed.A g7 str insrtr threaded shaft, part # 110003452 from lot 415830, was returned and evaluated against the complaint.Visual inspection confirmed the threaded tip of the shaft to be fractured.The fractured portion was not returned.A portion of the threaded face remains on the shaft.A review of the device history records identified no deviations or anomalies during manufacturing.Root cause of the event could not be determined.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
