Evaluation summary: the complaint product cartridge was not returned.The associated lens was returned posterior surface up in the lens case.Viscoelastic was observed on the qualified lens.One haptic was adhered to the anterior optic surface in dried viscoelastic.The lens was tilted in the well area and the optic edge is pressed against a post of the lens case.All product and batch history records are quality reviewed prior to product release.A qualified cartridge and lens combination was used.Handpiece information was not provided.Information was provided that indicated the use of a non-qualified viscoelastic.The root cause for the reported event of ¿tip broken¿ cannot be determined.The complaint product cartridge was not returned to be evaluated.Procedures and processes exist within the manufacturing environment that focus on protection of the cartridge tip.The manufacturer internal reference number is: (b)(4).
 
Event Description
A facility representative reported that during an intraocular lens (iol) implantation, the iol was loaded into the cartridge and then the cartridge broke while in contact with the patient.The procedure was completed.Additional information has been requested.
