(b)(4).Batch #: u93z8f.Investigation summary: the analysis results found that the el5ml device was received with no damage in the external components.The indicator was received in orange position and the lockout mechanism was non-functional.In order to evaluate the condition of the internal components, the device was disassembled and the ratchet pawl was found out of position causing the failure of the lockout feature.In addition, 15 clips were found inside clip track.No conclusion could be reached as to what may have caused this condition.Although there is no direct evidence of use error, the instructions for use do contain the following: "caution: firing the instrument over another clip or instrument can also damage a properly deployed clip, disrupt related vessels and structures, and damage the instrument." it should be noted that as part of our quality process all devices are manufactured, inspected, and released to approved specifications.Additional complaint information monitoring for potential safety signals will be conducted through complaint trending as part of post market surveillance.A manufacturing record evaluation was performed for the finished device batch number, and no non-conformance's were identified.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.
 
Event Description
It was reported that during a laparoscopic cholecystectomy, the clip was not fed into the jaws at the first firing.When the device was fired several times, the indicator soon showed the orange bar.The device was used on the cystic artery.Another device was used to complete the case.There were no adverse consequences to the patient.No further information is available.
