A facility reported the side of the mayfield skull clamp that holds the single pin was unstable when engaged in the patient's skull.It is unknown if there was patient involvement or if the device failure led to patient injury or surgical delay.

(b)(4).Device history record (dhr): the dhr shows no abnormalities related to the reported failure.Failure analysis & root cause analysis: mayfield skull clamp was received with the mayfield 2 lock having up and down movement and the teeth were grinding when rotated.Complaint not confirmed via inspection of the unit.Evaluation found when the clamp is positioned properly under pressure, it would not slip.Unit required general maintenance and cleaning.No further investigation required based on the acceptability of risk and no adverse trends identified.This will be monitored and trended going forward.
