The hcu 30 showed the error ¿1004-2 main heater temp.Sensor error¿.According to the "describe event of problem" grid the getinge field service technician (fst) removed the actuator from 3 way valve.The fst cleaned 3 way valve pin and did lubrication, after that the actuator was reconnected.The most probable root cause for the reported failure was the malfunction of the 3 way valve.No more failure was detected.Function tests all passed.Similar failure "error 1004" was already investigated in a similar complaint no.(b)(4) on 2014-10-02 with life cycle engineering (lce) investigation report no.(b)(4): the most probable root cause of the reported malfunction is a slight oxidation layer is inside the 3-way valve.The occurrence rate was calculated for the reported issue and it was determined that this is not a systemic issue.Therefore, no remedial action is required.The review of the non-conformities during the period of 2010-10-08 to 2020-11-30 does not show any non-conformity in regard to the reported product and failure.Thus the reported failure "error 1004-2" could be confirmed.It is unknown when the event occurred, was the device being used for treatment or diagnosis of the patient.The hcu 30 which was used, was responsible for this complaint.The occurrence rate related to the reported issue is currently being monitored as part of maquet cardiopulmonary¿s trending program and additional investigations or corrections will be implemented in case of adverse trending.
