Event Description
Information was received from a healthcare provider (hcp) regarding a patient receiving 2000 mcg/ml of lioresal at 609.8 mcg/day via an implantable pump.It was reported the patient heard their alarm going off and began having withdrawal symptoms on (b)(6) 2020.Upon interrogation it was confirmed the empty pump alarm went off.The patient stated the medical staff never told him his alarm date and said they would call him to schedule his refill appointment but they never called and that is how the pump went empty.The pump alarm was silenced and the hcp programmed the patient's dose to 400 mcg/day since the patient has been without drug for a little over a week.The hcp planned to bring the patient back the next week to titrate them back up to his current dosage.
