The customer returned asset scope was returned to the service center.The evaluation found the adhesive from the forceps/elevator cover at the distal end was missing.Additionally, control knob movement was loose.Minor cosmetic wear was noted at the light guide tube; buckle and scratches.A review of the scope repair history indicated the asset was last service/inspected (met standard specifications) on (b)(4) 2020.The scope was repaired back to specifications and returned to asset inventory.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.

As part of the investigation, olympus followed up with the user facility to obtain additional information but with no results the legal manufacturer performed the device history records for this device and all records indicated that the product was manufactured according to all applicable procedures and met final product release criteria.No abnormalities were found.The investigation was completed by the legal manufacturer and determined that there is no manufacturing, material or processing related cause for this failure mode.The forceps cover adhesive peeling/missing was likely due to external force such as strong rubbing on the part in normal use including reprocess that led to the occurrence of the phenomenon.As stated on the ifu and as a preventive measure, the user manual states: inspect the external surface of the entire insertion section including the bending section and the distal end for dents, bulges, swelling, scratches, holes, sagging, transformation, bends, adhesion of foreign bodies, missing parts, protruding objects, or other irregularities.Olympus will continue to monitor complaints for this device.
