During a vt ablation with tacticath se, the catheter was advanced via a retrograde aortic approach into the left ventricle for ablation on the posterior wall.During positioning of the catheter, the catheter advanced through the mitral valve and subsequently became entangled with the chordae tendonae.While trying to pull the catheter back, high force was noted on the tacticath se.As a result, a structual physician was called to consult on trying to remove the catheter and introduced a multipurpose catheter to loosen the hold of the chordae.During this, further manipulation was attempted on the tacticath se by the electrophysiologist and the catheter was freed without consequence to the patient.No further ablation was done and post-testing showed a successful procedure with no adverse consequences due to the entaglement of the catheter.
