Product event summary: the device was not returned for analysis, however, performance data collected from the device was received and analyzed.Analysis of the device memory was performed and no anomalies were found.If information is provided in the future, a supplemental report will be issued.
 
Event Description
It was reported that the right ventricular (rv) lead exhibited low impedance, increased thresholds and low r waves.A lead header interface issue was suspected with the possibility that blood ingress into the implantable pulse generator (ipg) header had occurred or that the lead sealing ring was damaged.Increased battery currents were also measured.The ipg and rv lead remain in use.No patient complications have been reported as a result of this event.
 
Event Description
It was further reported the lead was scheduled for explant and the ipg will be changed also.

Event Description
It was further reported that the implantable pulse generator (ipg) and lead were explanted and replaced.No patient complications have been reported as a result of this event.
