The product has not been returned for analysis.Complaint history and product history records were reviewed and documentation indicated the product met release criteria.Root cause has not been identified.There have been two other complaints in the reported lot number from the same facility.The manufacturer internal reference number is: (b)(4).
 
Event Description
A physician reported, while using the cartridge during intraocular lens loading the edge of the cartridge broke and the iol's haptic broke.There was no impact to the patient.Additional information is requested.
 
Manufacturer Narrative
The previous report submitted for this event contained an error in h.1.- ¿summary report¿ was inadvertently selected.After a recent systems update, a system error caused the inadvertent additional selection of ¿summary report¿ in h.1.On a select number of reports.The error, which was limited only to the h.1.Field, was promptly identified and quickly rectified.Correction smdrs are being filed for the impacted reports.This smdr is correcting that error for this event.The manufacturer internal reference number is: (b)(4).
 
Manufacturer Narrative
The used complaint company cartridge was not returned.One unopened company cartridge was returned in a 10-count carton.The unopened company cartridge was pulled from each returned carton.The two returned unused company cartridge was evaluated.Company cartridge was opened and microscopically examined with no damage observed.The company cartridge was functionally tested per the direction for use dfu.No lens or cartridge damage was observed after the lens delivery.The company cartridge was cleaned for further evaluation.Topcoat dye stain testing was conducted with acceptable results.Qualified associated products were indicated.No determination can be made without physical evaluation of the complaint sample.The company cartridges returned for the reported lot was evaluated.Functional and dye stain testing was conducted with the unopened sample with acceptable results.The manufacturer internal reference number is: (b)(4).
