Event Description
A facility reported that the head holder on the mayfield infinity skull clamp (a1114) felt too loose when clamped to the head of a patient.No patient injury or surgical delay was reported.

Device history record: the dhr shows no abnormalities related to the reported failure.Mayfield infinity skull clamp (a1114) was returned for evaluation.The reported complaint was confirmed via inspection of the device.The index knob was loose and unit needed new components to replace worn internal parts.
