The results of the investigation are inconclusive since the device was not returned for analysis.In addition, no log files were received.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.Based on the information received, the cause of the reported incident could not be conclusively determined.
 
Event Description
The night following an atrial fibrillation (af) ablation procedure, the patient experienced a transient ischemic attack (tia).An mri revealed a 2mm ischemia zone on the left hemisphere (inferior).The patient remained in stable condition over night and was released the next morning.The following day, the patient returned to the hospital with a recurrent arrhythmia and some neurological symptoms.Another mri was performed, which showed no additional findings.At this time, the neurological symptoms had resolved.The patient was discharged and remains in stable condition.It was noted that the patient was in stable condition before, during and immediately after the ablation procedure.A transesophageal echo was not performed prior to the procedure, and no anticoagulants were given prior or following the procedure.There were no performance issues with any abbott devices.
