Event Description
It was reported that a transfer set separated (not further specified).It was further reported that there was leakage from the twist clamp of the transfer set.The nurse detected this issue at the hospital room during continuous ambulatory peritoneal dialysis (capd) treatment.It was further stated that the transfer set was kept clean without using additional disinfectant.There was no patient injury or medical intervention associated with this event.No additional information is available.

Additional information: d9, h3, h6.H10: one actual sample was received for evaluation.A visual inspection with the naked eye and magnification was performed which noted the silicone tubing was twisted inside the main body of the twist clamp.The occlude feet were intact and no other issues were observed.Clear passage testing was performed which revealed no flow through the set.Leak testing was performed with no issues noted.The reported condition of leak was not verified, however, the sample failed to meet specification due to no flow through the set.The cause of the condition could not be determined.Should additional relevant information become available, a supplemental report will be submitted.
