Event Description
It was reported that a packaging issue occurred.When the box for a 2.25mm x 15mm nc emerge balloon catheter, the lot number for the device inside the box was different than the lot number on the box.The box tab was not loose prior to opening.The procedure was completed with a different device.No patient complications were reported.
