At this time product has not yet been returned.An extended investigation has been performed for the reported complaint and it has been determined that there was no indication that the product did not meet specification.The dhrs (device history review) for the libre reader was reviewed and the dhrs showed the libre reader passed all tests prior to release.If the product is returned, a physical investigation will be performed and a follow-up report submitted.The date the incident occurred is unknown.The date entered is the date abbott diabetes care became aware of the event.All pertinent information available to abbott diabetes care has been submitted.
 
Event Description
A customer reported that the adc freestyle libre 2 reader button or switch sporadically it goes on its own and values are often not displayed and that the reader was not working.As a result, the customer experienced symptoms of disorientation and a loss of consciousness and was unable to self-treat however, no further information was reported.There was no report of death or permanent impairment associated with this event.

Reader (b)(6) has been returned and investigated.A visual inspection was performed on the returned reader and no issues were observed.A built-in reader test was performed; all tests passed and functions cycled normally.No malfunction or product deficiency has been identified.All pertinent information available to abbott diabetes care has been submitted.
