The results of the investigation are inconclusive since the device was not returned for analysis.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.Based on the information received, the cause of the reported incident could not be conclusively determined.
 
Event Description
During a premature ventricular contractions ablation procedure, the device became entangled with the patient's anatomy.The catheter was oriented at a three-o'clock direction of the mitral valve in the basal left ventricle.While inserting the catheter through the trans-aortic approach and when searching for the target lesion, the tip of the catheter became entangled.At this time, mapping was being performed with the catheter being rotated at six o'clock direction of the mitral valve in the basal left ventricle.After that, the catheter became unable to be manipulated properly and could not be removed from the patient.The contact force was confirmed along with the heart beat, and it is assumed that the catheter tip was entangled with some kind of the tissue in the heart.The patient was transferred to cardiovascular surgery and a thoracotomy was performed to remove the device.After successfully removing the catheter from the patient, a hiatus was found in another part of the left ventricle and it was surgically repaired.The reported catheter was discarded by the hospital.The physician does not attribute this event was due to any abbott device.
