Event Description
|
The reporter indicated that the doctor observed a particulate on a 13.2mm, micl13.2, -9.5 diopter, implantable collamer lens and chose to use the backup and the problem resolved.The reporter stated "material stuk to lens after removing from packaging, tried to clean off.Was unable to".In the reporters opinion the cause of this event is the device.There was no patient contact.

H3- device evaluation: lens was returned in liquid, in lens vial.Visual inspection found no visible damage to the lens.No particulate was noted stuck on the lens.H6- device history record (dhr) review: based on the results of the investigation, all released devices from the associated work order (s), including the suspected device, have been manufactured within established process parameters; and there is no indication that the manufacturing and processing of the device contributed to the complaint issue.Claim # (b)(4).
