Event Description
|
Olympus was informed that during an unspecified procedure, the loop wire at the distal end of two hf resection electrodes of the same lot broke off and fragments fell into the patient.It was only possible to locate and remove the broken fragments by using an ultrasound device.However, this led to complications and resulted in a significant prolongation of the procedural time.There are no reports of any complication after the surgery.

The suspect medical devices were not returned to the manufacturer for evaluation/investigation since they were reportedly discarded by the user facility.Therefore, the exact cause of the patient's outcome and the reported phenomenon could not be conclusively determined and is being judged as unknown.However, the reported failure is a known phenomenon and can very likely be attributed to component failure caused by use-related wear and tear.A material or quality problem can be excluded since a manufacturing and quality control review was performed for the affected lot number of the resection electrodes without showing any abnormalities.The case will be closed on olympus side with no further actions.The reported event/incident will be recorded for trending and surveillance purposes.
