The subject device was received and evaluated at olympus (b)(6) service center.Inspection found the device scope detection does not work.Scope detection skips.Furthermore, the device base plate and lid were both found corroded.Investigation is ongoing.This report will be supplemented accordingly following investigation.
 
Event Description
As reported, during device service inspection it was found that the device scope detection does not work.There is no patient involvement associated on this reported event.No user injury reported.Prior to the event reported, the device was used for an endoscopy examination which was completed successfully with the same device.There was no extension of the procedure and no injury to the patient.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 5 years since the subject device was manufactured.Based on the results of the investigation, and it is likely that the parts of the slide switch were worn out due to repeated use or accidentally broken down.If the corners of the slide switch parts become rounded, it is probable that the switch became difficult to push in when the scope was inserted, making it impossible to detect it.Olympus will continue to monitor the field performance of this device.
