An investigation has been initiated based on the reported information.Upon completion of the investigation, a follow-up report will be submitted.
 
Event Description
A facility reported that the swivel lock on the mayfield modified skull clamp (a1059) was broken, ratchet mechanism was sticking and does not open or close easily.It is unknown if there was patient involvement; however, no patient injury or surgical delay has been reported.
 
Manufacturer Narrative
Updated fields: d4, d9, g3, g6, h2, h3, h6, h10.The mayfield skull clamp (a1059) was returned for evaluation: failure analysis - the investigation of the returned device verified the reported complaint.Unit received with the lock having rotational and lateral movement and a residue buildup was present.New components was added to replace worn internal parts along with general maintenance and cleaning.Root cause - based on the evaluation by integra service and repair, the root cause is most likely normal wear over time, in addition to the need for routine preventive maintenance.The unit is 11 years old, well beyond integra's recommended 7 year life cycle (manufactured 2010) no further investigation required based on the acceptability of risk and no adverse trends were identified.This will be monitored and trended going forward.

The mayfield modified skull clamp (a1059) was not returned for evaluation; therefore, an evaluation of the device could not be performed.Lot number information has been provided; therefore, device history record (dhr) was reviewed, and no anomalies were found.The root cause(s) of the reported issue could not be determined.The reported condition is likely caused by improperly handling/ wear and tear of the device.If additional relevant information becomes available in the future, this complaint will be reopened, and the respective evaluation performed.Trends will be monitored for this and similar issues.At present, we consider this complaint to be closed.
