Event Description
It was reported that during a system upgrade procedure, the patient's coronary sinus vein ruptured.The left ventricular (lv) lead had prolapsed from its position, which resulting in the pacing causing diaphragmatic stimulation.Pericardial effusion occurred and the operation was suspended due to the fluid accumulation.The lv lead was removed and the patient was hospitalized for several days.The lv lead was replaced with an epicardial lead.No further patient complications have been reported as a result of this event.
