Event Description
Medtronic received information regarding a pipeline that was noted to be bent at the distal tip and could not be deployed properly.It was reported that the devices were prepared and the catheter flushed per the instructions for use (ifu).When the pipeline was first deployed the distal tip was found to be bent and could not be deployed successfully.The pipeline was less than 50% deployed when the deformation was observed.The pipeline was resheathed 3 or more times without success in resolving the pipeline kink preventing full apposition.No other manipulation or other devices were tried.The pipeline was finally resheathed and removed together with the phenom microcatheter.Both devices were replaced to complete the procedure.No patient information was reported.The aneurysm was located in the internal carotid artery (ica) cavernous segment.The aneurysm max diameter was 13mm and the neck diameter was 8mm.Additional information received reported that the stent failed to open at the distal end, and there was some friction during delivery.There was no damage observed to the phenom catheter.
