The device was returned, and an initial evaluation was conducted by olympus; however, investigation is ongoing.During the initial evaluation, the user report was confirmed, the unit failed the scope leak check due to a hole in the device.Additionally, the insertion tube had been cut, which was also contributing to the leaking issue.It was then noted that the plastic cover had worn out adhesive with minor dents and scratches.If additional information becomes available following the device evaluation, a supplemental report will be filed.
 
Event Description
The customer reported that their evis exera iii gastrointestinal videoscope got a hole through the bending cover during an unspecified procedure.Additional details relating to the patient and the event have been requested, but no are unknown.There was no patient harm or consequence reported as a result of this event.
 
Manufacturer Narrative
Upon further review this is not a reportable malfunction.Per the legal manufacturer there is no potential for this issue to cause or contribute to death or serious injury if the malfunction were to recur.
