Manufacturer Narrative
Siemens has completed an investigation of the reported event. The root cause was determined to be a hardware error. The investigation was performed considering complaint description, cs reports, system history, and system log files. The live monitor of the sensis system went black during a scheduled interventional study.Although the system was booted up, no vital signs were displayed. As a result, the procedure was continued and finished using an alternative system. The analysis of the log file confirms an ongoing examination with a connected and monitored patient. This system consists of 2 individual pcs.One of them still displaying the vital signs. It could also be determined that the connection to the artis system was interrupted, and the user restarted the sensis system to fix the problem - without success.Siemens service was called for troubleshooting. As no system issue was found during the service intervention, the entire hardware of the signal input box (sib) was replaced as a preventive measure. After the replacement, the problem did not recur. No further details could be derived from the available information and no clear cause could be identified.A possible general error that would require corrective measures of the installed base could not be determined by the investigation. After detailed investigation, the incident is not classified as a reportable event as neither serious injury, death nor an unexpected, prolonged hospitalization of the patient or any other person occurred or could be expected.
