|
The customer, a syncardia authorized distributor, reported that the freedom driver was defective, however there was no additional information provided.There was no reported adverse patient impact.The customer also reported that the patient was subsequently changed to a backup driver.

The freedom driver will be returned to syncardia for evaluation.The results will be provided in a follow-up mdr.Ce (b)(4) initial.

The driver's alarm history was reviewed and revealed a 2d alarm code which is recorded when the secondary motor is engaged.Visual inspection of the interior components of the driver revealed that the secondary motor was out of bottom dead center (bdc) position, the scotch yoke was cracked and the primary motor was binding when turned by hand and was covered in debris.Upon review of the driver's alarm history and visual inspection, it is reasonable to concluded that the freedom driver exhibited an alarm which caused the patient to switch drivers.The root cause of the fault alarm was accumulated debris which caused the primary motor to bind up, leading to the secondary motor operation and annunciated alarm as designed.This issue will be monitored and trended as part of the customer complaint process.Syncardia has completed its investigation and is closing this file.(b)(4) follow-up report 1.
