The device was returned, and an initial evaluation was conducted by olympus; however, investigation is ongoing.During the initial evaluation, the user report was confirmed, the scope connector port was damaged.As noted, the sensor on the scope detector was damaged, causing no image to appear.Additionally, there was cosmetic damage noted on the front panel, and the lamp life meter was reading at over 500+ hours.If additional information becomes available following the device evaluation, a supplemental report will be filed.

The customer originally returned their evis exera iii xenon light source because it was experiencing a scope connector port issue during a diagnostic procedure.According to the initial reporter, the procedure was completed by using a travel cart unit, and there was no negative impact to the patient.During the device evaluation, it was discovered that the scope detection sensor had failed.This report is being submitted to capture the failed scope detection sensor.
