Manufacturer Narrative
The device was returned to olympus for evaluation and the customer¿s allegation was confirmed.Inspection of the returned device found that the rubber on the pink probe was missing a chip.In addition, the bending section cover glue had a cementing defect, the control unit body and cover was scratched and the ultrasonic universal cord was kinked.There was corrosion on the air/water and stopper nuts for the control unit.The investigation is ongoing and a supplemental report will be submitted upon completion of the investigation.
 
Event Description
The customer reported to olympus, the ultrasound head on the ultrasonic gastrovideoscope had a small hole, near the distal end on the sheath.Inspection of the returned device found that the rubber on the pink probe was missing a chip.The procedure was completed and the patient was transferred home.There were no reports of patient harm associated with this event.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation, a missing piece / chip / parts fell off on the probe unit.It was determined that these conditions occur due to a physical stress by dropping and/or knocking the affected part to a hard surface/object and applying a chemical stress during the cleaning/sanitization process.The instruction manual identifies the following related verbiage: ¿inspection of the endoscope.3.Inspect the external surface of the entire insertion tube including the bending section and the distal end for dents, bulges, swelling, scratching, sagging, deformation, excessive bending, protruding objects or other irregularities.¿ olympus will continue to monitor field performance for this device.
