Event Description
Medtronic received information that when the physician started to deploy the coil in the aneurysm wall the axium detached in microcatheter before full deployment in aneurysm wall.The patient was being treated for a ruptured saccular aneurysm with a max diameter of 5.6mm and a neck diameter of 3mm.The location was the middle carotid artery.Patient's blood flow was high and vessel tortuosity was moderate.It was indicated the devices were prepared according to the instruction for use (ifu).Continuous flush was administered.  the coil was removed by carefully pulling the whole system, like catheter, intermediate catheter. the physician rotated the delivery pusher during the procedure.No patient symptoms were reported.  ancillary devices: headway 17 microcatheter.

Additional information received reported that no detachments had been made, and no kink/damage to the pushwire was observed during the procedure.

Event Description
The pushwire was not bent or broken, there was no friction or difficulty during delivery.The physician rotated the pushwire during delivery.No patient symptoms were reported. .
