|
It was reported that within seven minutes of starting treatment with a gambro cartridge, the venous line started leaking out blood.It was further described that the venous leur lock, from the blood cartridge, appeared to be tightened, however, when rotated further, it became loose.The amount of blood loss was reported to be approximately 5ml.A prophylactic treatment with vancomycin was administered intravenously.There was no patient injury or medical intervention associated with this event.No additional information is available.

H10: the actual device was not available; however, one (1) photograph and eight (8) retained samples were provided for evaluation.Visual inspection of the picture identified a damage in the venous patient connector.The reported condition was verified.The cause of the condition was determined to be a manufacturing issue.A nonconformance has been opened to address this issue.Visual inspection of the eight (8) retained samples, leak testing and functional tests did not identify any abnormalities that could have contributed to the reported condition.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
