The freedom driver will be returned to syncardia for evaluation.The results will be provided in a follow-up mdr.Ce (b)(4) initial.
 
Event Description
The customer, a syncardia authorized distributor, reported that the freedom driver exhibited a fault alarm while supporting a patient.There was no reported adverse patient impact.The customer also reported that the patient was subsequently changed to a backup driver.
 
Manufacturer Narrative
The driver's alarm history was reviewed and revealed multiple "19" (bottom pressure too low) alarm codes.The driver did not pass functional testing as it exhibited a continuous alarm.A "19" fault code is usually an indication of a malfunction of the u22 pressure sensor on the main printed circuit board assembly (pcba).A known functioning main pcba was installed in the driver and retested with no alarms.Additionally, the suspect main pcba was installed in a test driver which immediately exhibited an alarm when tested.The root cause of the customer-reported fault alarm was determined to be a malfunction of the u22 pressure sensor on the freedom main pcba.This issue will be tracked and trended as part of the customer complaint process.Syncardia has completed its investigation and is closing this file.(b)(4) follow-up report 1.
