During a product maintenance visit by a field service engineer (fse), the fse noted that the olympus endoscope reprocessor was experiencing a functionality problem.According to the report, the device would not begin the reprocessing cycle after the water was replaced and the button was pressed.There was no patient involvement during this event.

This event was discovered during a routine on site visit performed by an olympus fse.While further evaluating the device, the fse noted that in addition to not cycling properly, the processor was also leaking.The fse then decided to replace the 3-port value connectors as well as the o-rings, and clean the front panel.Following those corrective steps, the unit was verified using device standards, and successfully passed all functional tests.At this time, the device is not scheduled for return.However, should additional information become available, a supplemental report will be provided.
