This is filed to report material separation.It was reported during preparation of the steerable guide catheter (sgc), the flush port cracked, and a portion broke off.Therefore, the sgc was not used and was replaced.There was no clinically significant delay in the procedure.No additional information was provided.

All available information was investigated, and the reported crack and material separation of the flush port were confirmed via returned device analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot.Additionally, a review of the complaint history identified no similar complaints reported from this lot.Based on the information provided, the reported material separation of the flush port was due to the crack of the flush port on the hemostasis valve.The investigation determined the reported crack of the flush port as a potential product issue.Abbott will continue to trend the performance of these devices.D4: lot number was updated.
