Event Description
|
Information was received from a manufacturer representative regarding a patient who was implanted with an implantable neurostimulator (ins).It was reported that the caller stated they were in or doing new implant and when hcp is attempting to tighten set screw for the bottom port, it is not clicking and appears to be missing.Caller confirmed they didn't see the set screw in any of the packaging (as if it had come out of the header block). technical services (tss) reviewed that the only other kit that contains screws are extension kits but caller didn't have any of those.Tss reviewed that there could be an issue with the hole/port where the set screw sits as well which may cause raise concern with the ins header block itself.Tss reviewed it would be advised to use another ins.
