The device was returned to olympus for evaluation.The evaluation uncovered moisture has entered the device, does not show image due to a defective dr board and the video connector socket does not lock in video connectors.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported to olympus, flooded moisture has entered the device on the evis exera iii video system center during an unspecified procedure.There was no harm or user injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information, based on the legal manufacturer's final investigation.It has been over 8 years, since the subject device was manufactured.A review of the device history record found no deviations, that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation.Liquid invasion to the device dr board damage.And excessive force applied to the video connector socket were observed.And were likely caused, by mishandling against instructions for use.Olympus will continue to monitor field performance for this device.
