In communication with the customer by tac (technical assistance center) support to advise customer, it was determined (the customer stated) that the device is working and will not be sending in for evaluation.The customer did not provide further information as to how the issue reported was resolved.It is unknown at this time the cause of the reported issue.Supplemental report(s) will be submitted should any relevant new information is available and or received.Investigation is ongoing.This report will be supplemented accordingly following investigation.
 
Event Description
Customer reported the lightsource completely shuts off and will have to restart.The issue occurred during an unknown event.There was no patient harm or injury reported due to the event.No user injury reported.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the approved final investigation.Sections e2 and g2 were corrected.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the reported issue could not be determined.Olympus will continue to monitor field performance for this device.
