Event Description
During an atrial fibrillation / atrial tachycardia procedure, damage was noted to the catheter splines.Mapping was done in the right atrium and while the catheter was being retracted from the patient, resistance was felt.The catheter was advanced back into the patient and an x ray revealed it was looped over on itself.After taking it up above the sheath tip and retracting it again and the catheter was removed from the patient without issue.Visual inspection revealed the two middle splines of the catheter were damaged, with one spline showing the outer grey coating pushed up to the distal end of the catheter along with the electrodes.One electrode wire had snapped.The physician thought the snapped wire was due to the catheter being snagged on the short sheath.
 
Manufacturer Narrative
One bi-directional, curve d-f, sensor enabled, advisor hd grid mapping catheter was received for evaluation.Electrodes 5-10 were displaced and the pellethane tubing on splines b and c were corrugated and torn.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.The cause of the torn and corrugated pellethane tubing and displaced electrodes remains unknown.
