Event Description
The customer reported to olympus, the evis exera iii xenon light source had no picture and humidity was observed in the connector during preparation for use.Upon inspection of the customer¿s returned device, scope communication error b30 was observed, and traces of water/fluid was observed in the socket.This report is being submitted for the malfunction found during evaluation.There was no patient involved in this event.
 
Manufacturer Narrative
The subject device has been returned to olympus for evaluation.During evaluation, error b30 (scope communication error) was observed.It was noted that the reported issue was caused by liquid in the scope socket.It was also noted, traces of water/fluid were observed in the scope socket, causing humidity in the connector.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation, there were traces of water/fluid found in the s socket.The cause of water trace was the damaged s socket, therefore, the s socket needed to be replaced.Olympus will continue to monitor field performance for this device.
