The subject device has not been returned to olympus for evaluation.It was noted, the customer reached out to the field service engineer (fse) to resolve the issue.During troubleshooting, the central processing unit (cpu) was reset, the lcg was replaced but the light stayed on.Customer was advised to contact the fse once the aldohol was replaced again, to see if the light goes out.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported to olympus, the liquid chemical germicide (lcg) light of the endoscope reprocessor stayed on even after it has been reloaded.There was no harm or user injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Sections e2 and e3 were inadvertently selected in error.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the disinfectant replacement indicator not disappearing after replacing the disinfectant could not be determined.Olympus will continue to monitor field performance for this device.
