The subject device was returned to an olympus service center for evaluation.During inspection and testing, it was observed the socket of the co2 (carbon dioxide) insufflation connector was deformed.In addition, the left side of the front panel inside the device was damaged, resulting in the insufflation connector being detached from the front panel.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported the front panel of the subject device was cracked, and the gas cylinder interface was deformed.The device is an olympus loaner device.An olympus field service engineer (fse) further explained that the screw at the front panel connection was broken, and the connection thread was deformed, so the cylinder could not be installed normally.There was no patient or procedure involvement.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the cause of the deformed gas cylinder interface and socket of the co2 gas-injection connector could not be identified.Per the legal manufacturer, the other device defect included in the initial mdr (the screw at the front panel connection was broken) has no potential to cause or contribute to death or serious injury if the malfunctions were to recur.Olympus will continue to monitor field performance for this device.
