Event Description
Information was received from healthcare professional regarding a patient having c1/c2 posterior fixation and decompression.It was reported that use of 3 nm toque limiting handle on 2 nm driver shaft and set screws resulted in rounding of both the set screws and the driver shaft.There was no patient symptom reported.There were no further complications reported regarding the event.
