Event Description
It was reported that a minicap extended life pd transfer set had a "clicking sound" when the twist clamp was turned.This occurred during use of the device for peritoneal dialysis therapy.No disinfectant was used to disinfect the transfer set.The transfer set was in use for approximately 1.5 to 2 months.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

B5: upon additional information, fluid flow was observed with the clamp closed with the transfer set.The event occurred during an unknown process step.There was no report of patient injury or medical intervention associated with this event.The sample was received for evaluation.A visual inspection with the naked eye noted a broken occluder feet.Functional testing including leak, clear passage, clamp function and torque engagement testing were performed; leak and clamp function testing revealed a flow through the set.The reported condition was verified.Torque and clear passage testing were performed with no issues.The cause of the condition could not be determined; however, the damaged set is consistent with damage caused by exposure to chemical agents such as foreign solvents, dyes, or cleaning agents that damage the transfer set materials.A nonconformance has been opened to address this issue.Should additional relevant information become available, a supplemental report will be submitted.
