Event Description
According to the reporter, during a laparoscopic roux en y plus hiatal hernia procedure, while performing the hiatal hernia, the surgeon felt no pressure while suturing and the needle split in half and fell into the patient's cavity.The surgeon was able to locate the needle pieces with flat graspers.A new device was used to resolve the issue.There was no patient injury.
