The freedom driver will be returned to syncardia for evaluation.The results will be provided in a follow-up mdr.
 
Event Description
The customer, a syncardia authorized distributor, reported that the freedom driver exhibited a fault alarm while supporting a patient.There was no reported adverse patient impact.The patient was in the hospital in bed because of raised body temperature, with good blood pressure, and normal fill volumes and cardiac output values.The customer also reported that the patient was subsequently switched to a backup driver.
 
Manufacturer Narrative
Visual inspection found no evidence of secondary motor engagement.Visual inspection found a crack on the driver display cover.Damage is cosmetic and not related to the customer reported issue.No other damage to internal or external components was found.The driver underwent extended observation testing in an attempt to duplicate the reported fault alarm; no issues or alarms were observed during the observation test.The customer reported fault alarm could not be confirmed or replicated.No evidence of a device malfunction was found during the investigation.This issue will be monitored and trended as part of the customer complaint process.Syncardia has completed its evaluation and is closing this file.If new or additional information is received in the future, syncardia will file a follow-up mdr.(b)(4) follow-up report 1.
