The customer reported to olympus technical assistance center (tac), the endoscope reprocessor had a water flow issue.There was no harm or user injury reported due to the event.

The subject device has not been returned to olympus for evaluation.During a follow up call with the customer, the customer noted the unit is now working fine.The olympus representative advised to verify when the last time the water filter was replaced.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Section g2 was corrected to accurately identify the report source as a healthcare professional.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the definitive root cause of the weaker water supply flow in the reprocessing bin could not be determined.It was observed that the primary side of the water filter was dirty due to a water issue that occurred in the area of the hospital.It is possible that the dirty water filter caused it to clog, which may have resulted in a weaker water flow.Olympus will continue to monitor field performance for this device.
