Event Description
Medtronic received information that during the implant of this 26mm mitral mechanical valve, it was explanted and replaced with a 27mm mitral mechanical valve.The reason for the replacement was reported as sever regurgitation.  it was reported that a crack was noted at the junction of the 26mm machinal valve "braided annulus and the metal ring, a stream of blood flow was ejected from the crack with a width of about 6mm".No additional adverse patient effects were reported.
