Event Description
|
A facility reported that the tension on the mayfield skull clamp (a1059) released at the spring during a cervical case.They stated that they pinned up to the 60 reading on the clamp; however, the clamp subsequently failed and they had to change over (from pinning the head to not using disposable pins, and transferred to horseshoe) mid case.There was delay for 30 minutes with no patient injury reported.

Mayfield modified skull clamp (a1059) was returned for evaluation: device history record (dhr): the dhr was reviewed and shows no abnormalities related to the reported failure.Failure analysis - investigation showed that the reported fault was not confirmed.Although the device has passed all tests to manufacturer's specifications, we recommend a general preventive maintenance as the locking mechanism is clogged and requires cleaning and adjusting and replacing parts to bring the unit back to manufacturer's specifications.Root cause - evaluation found no device deficiencies that would have contributed to the reported complaint.The device has passed all tests to manufacturer's specifications, device received general preventive maintenance and cleaning services.Probable root cause is improper or suboptimal positioning of the skull clamp.).No further investigation is required based on the acceptability of risk and no adverse trends were identified.This will be monitored and trended going forward.
