This is filed to report a loose component.It was reported that on (b)(6) 2022, a mitraclip procedure was performed to treat a degenerative mitral regurgitation with grade of 4+.It was noticed that the cover that encases the gripper levers was extremely loose.The physician had to hold it down when raising and lowering the gripper.It was discussed as a team whether or not to get a new device.The physician wanted to continue with the same device despite it being too loose.The clip was implanted successfully, reducing the mr to a grade of 1-2.There was no clinically significant delay in the procedure and no adverse patient effects.No additional information was provided.

All available information was investigated, and the reported loose connection of gripper lever cover was confirmed.In addition, the gripper lever assembly tab was observed to be broken.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents reported from this lot.Based on available information, the reported loose or intermittent connection of the gripper lever cover was due to the observed broken gripper lever assembly tab.A cause for the observed broken tab could not be determined.There is no indication of a product quality issue with respect to manufacture, design or labeling.
