Event Description
It was reported via maude report mw5111784 that shaft break occurred.A 2.00mm x 12mm apex balloon catheter was advanced for use.The apex balloon catheter was inserted.A kinked was observed and the apex balloon catheter broke when it was removed from the patient.There were no patient complications nor injuries reported.
