Event Description
It was reported that the patient has a short circuit from impedances done at contact pairs 0-1.The patient is scheduled for an mri which is why the question came up.Radiology was advised that on-label mri could not be approved because of the impedance issue.Circumstances that may have led to the issue was unknown.The patient was programmed around the short by the hcp.The issue was resolved at this time.

Event Description
Additional information received from the manufacturer¿s representative (rep) reported the left lead was difficult to explant at the time of the new lead implant.Due to this the lead was still implanted and connected to the extension and was partially cut and not active or connected to the implant; it was floating.The new implant had a port plug superior and the lead/extension for the right psa in the inferior port 8-11.
